This notice explains how you can access and how your medical information may be used and released. Please give it a thorough read.

Care Compass Nursing, LLC values your privacy and confidentiality. We’ve put rules and guidelines in place to safeguard and limit the use of your protected health information. Your name, address, phone number, social security number, email address, diagnosis, and any other similar information you provide during treatment may be used to identify you. Protected Health Information (PHI) is medical information about your care and treatment that is related to you. PHI could exist in an electronic or paper format.
The federal and state governments also regulate home health care to guarantee that we provide high-quality services, that you receive them, and that your protected health information is kept private and safe.
COLLECTION AND USE OF WRITTEN AND ELECTRONIC PORTECTED HEALTH INFORMATION
We must gather information about you and your medical needs from you and others in order to deliver services to you (e.g., insurance companies, hospitals, physicians, family members, other caregivers).
Care Compass Nursing, LLC will not sell your protected health information in her capacity as your nurse. We must exchange information about you and your medical needs for treatment, payment, and our health care operations in order to give you the best care possible. In the course of treatment, PHI may be shared with members of your interdisciplinary team, doctors, hospitals, other healthcare providers, family members, close friends, and other individuals you designate, such as clergy, private caregivers, pharmacists, suppliers of medical equipment, and others. We will use and share your information for Care Compass Nursing, LLC’s health care operations and quality improvement in order to enhance our care, lower expenses, or better train our staff.
Some examples of how we use your protected health information include:
• To stay in touch with your doctor or other interdisciplinary team members to make sure that your needs are being fulfilled and that your treatment plan and instructions are maintained current.
• To assist make sure we are delivering high-quality care, for evaluation, teaching, and training purposes inside our organization, including to students or trainees under our supervision.
• To inform you of or suggest potential courses of action, goods, or services that other Care Compass Nursing, LLC offices or outside vendors might provide that might be of use to you. • The few non-employees we do business with, known as Business Associates, may help us with your care, collecting payment, or running our everyday medical practices. Our Business Associates could be, for instance, a company that provides response services or records storage. Each Business Associate agrees to abide by the same rules as Care Compass Nursing, LLC by signing a contract.
We need your signature on a separate HIPAA Authorization form before we can disclose any information that isn’t legally necessary, unrelated to your care, payment, or health care operations. For instance, we will get your consent in writing before disclosing information if a lawyer contacts us requesting copies of your medical records. Furthermore, certain PHI is extremely safeguarded, such as data pertaining to drug misuse, STDs, or diagnoses like HIV/AIDS.
We will collaborate with the person you have nominated as your medical power of attorney, legal guardian, or someone else authorized to make decisions on your behalf. • If we have good grounds to think that you may be a victim of abuse, neglect, domestic violence, or other crimes, or if we want to protect you from harm or danger. • For the purpose of public health initiatives and to prevent a major or impending threat to your health, safety, or the safety of others.
• In the case of a natural disaster or other safety hazard to you or your house, emergency preparedness can assist assure your safety.
YOUR PRIVACY RIGHTS:
Although your medical file and PHI are the property of Care Compass Nursing, LLC the information belongs to you. You have the right to:
• Be aware of and access the data regarding your care that Care Compass Nursing, LLC keeps on file in your medical file. You can request print or electronic copies of your medical file, or review the material in person. To schedule your visit or obtain copies of your file, you must contact us in writing and allow enough time. You can access your records in an electronic format if they are kept on an electronic platform. You can also request that we give your medical record to another party. You can assign someone to view the records on your behalf if you are unable to do so yourself. Normally, we respond to requests for access to your records in less than 30 days. Additionally, we could impose a fair copying fee for our services.
• Request that we contact you about your health concerns at a specified number (your office or cell phone number, for instance, or at an address other than the one we have on file). If possible, we will comply with your request.
• Request that specific individuals not be granted access to your protected health information, including restricting information shared with your health plan in the event that you or a representative have paid for services in full out of pocket. Kindly inform your office director or clinical manager of any requests or limitations.
• Get informed if we suspect that your PHI has been accessed without authorization (breach). If there is a breach, we will try to reach you over the phone and send you a written notice by email or first-class mail within 60 days. We will inform you of the situation and our corrective actions. We will also follow any state specific laws that may have different requirements.
• Request that any information, whether in paper or electronic media, that you feel is inaccurate or lacking be changed from our files. Please ask your clinical manager or director for the “Request to Amend My Protected Health Information Form” form in order to make an amendment request. We will update your data and comply with your
request in the majority of cases; but, there are a few circumstances in which we might not be able to.
• Ask for a list of all the people who, within the previous six years, obtained your protected health information without your consent (accounting). The list explains the date, who we shared it with, and why. Releases to lawyers, law enforcement, and government organizations might be part of it. Please get in touch with the director of your local office to acquire a copy of this list.
VISUAL IMAGES
Photographs, videotapes, and digital or other images may need to be recorded to document your care. Care Compass Nursing, LLC retains ownership rights to these photographs, videotapes, and digital or other images. We will store them in a secure manner to protect your privacy. You have the same rights outlined above to see these images or to obtain copies.
Care Compass Nursing, LLC may send you emails with information regarding your care. We take precautions to ensure the confidentiality, security, and privacy of email correspondence when we do this. Any email pertaining to your PHI is also appended to your medical record.
We shall refrain from sending sensitive medical information via insecure email when discussing HIV/AIDS, drug misuse, or STDs. If there are additional kinds of data that you would prefer not to be sent over email. We’ll try our best to comply with your request.
You or any other parties who would like to email Care Compass Nursing, LLC regarding your care should only use an email address that ends in (carecompassnursing.com). We are unable to ensure that emails sent to other addresses will be received or will be secure. Care Compass Nursing, LLC cherishes our relationship with you and whenever feasible, would want to speak with you over the phone or in person. We promise to reply as soon as possible if you contact us via email. Email should not be used for urgent or time-sensitive topics.
CHANGES TO THIS NOTICE
We might need to revise or amend this privacy notice from time to time. The revised Notice will be posted on our website (www.carecompassnursing.com) and made available at your service office. Please send us an email at any time if you would like a paper copy of this notice.
CONTACT FORM DATA
The information you provide on our contact form is sent to us by email. This is so that, rather than keeping them in a moderation queue, we can instantly identify and approve any follow-up comments.
We also keep the personal data that users enter in their user profiles when they register on our website, if any. All users are able to view, modify, or remove their personal data at any moment; however, they are unable to modify their username. Administrators of the website can view and change that data as well.
